Automatic Creation of Addendum Sheets.
All information is printed on the form in 11 point type, bold font for clarity and neatness.

Any information that does not fit on the form is automatically printed on an attached Addendum Sheet.

Some of the questions on the forms can require more of an explanation than will fit on the form.

The system will allow for this information and attach this to the addendum sheet identifying which sections are included

There is even a space for additional information that you feel the Board should be aware of in each incident

Intuitive Use
The Addendum Sheet means that the user does not have to try to 'squeeze' the information into to limited space provided on the paper forms.
